The objective of writing a request for leave for temporary or permanent leave is to get enough time off to deal with their personal issues that you are dealing with. It will mainly depend on the reason you apply for leave, whether or not you will receive a letter of confirmation or not. 

Whether the letter is formal, or not, we advise putting the reason for the leave in the first paragraph of the letter's body. In the final paragraph, you need to summarize the purpose of your letter again and suggest a suitable course of action to follow. Do not indent the paragraphs. But instead, leave an empty line between each paragraph.
