How do I write an employee performance review (EPR)? What is employee evaluation form?

It’s hard to know what questions to ask employees during a performance evaluation or employee performance review (EPR). Even once you’ve decided what to ask, you need to organize the questions you have and collect the feedback. Employment Performance reviews are important for every HR/HRM department. Not only for ensuring the optimizing the operation of a business or organization, but also to motivate and support employees by helping them in their career plan.

This EPR classifies as a sample Human resources (HR) document. Building a business has many steps and requirements, and one crucial component of starting and getting a business to grow that often gets overlooked is HR. Having an HR manager or department within your company requires effort and knowledge, and, depending on the size and industry, might become quickly overwhelming. 

Within an organization, the Human resources department manages its employees, from the beginning: by selecting and recruiting staff (sifting through resumes, job applications and conducting interviews), onboarding and training them, do performance reviews (KPO setting, annual review, etc), all employment-related paperwork, and finally also arranges the termination of the employees. They need to oversee the organization's fair employment practices, HR Handbook, workplace safety, benefits, payroll, and vacation time.

What should be included in an employee evaluation form?

An effective employee performance review consists of:

  • Aligning individual goals with those of the business.
  • Providing clear communication between work responsibilities and objectives.
  • Identifying training needs and possible career plans.
  • Encourages positive relationships between employees and management.
  • Performance reviews are also tied to business planning since they often coincide with pay changes and promotions.

Often organizations only perform annual performance reviews that are related to the bonus. By conducting informal, monthly reviews can ensure management is better informed, issues are addressed as they arise. It also ensures a more positive and clear relationship.

  • Quality of Work:
    • Work is completed accurately (few or no errors), efficiently and within deadlines with minimal supervision
  • Attendance & Punctuality:
    • Reports for work on time, provides advance notice of need for absence 
    • Reliability/Dependability:
    • Consistently performs at a high level; manages time and workload effectively to meet responsibilities
  • Communication Skills:
    • Written and oral communications are clear, organized and effective; listens and comprehends well  
  • Judgment & Decision-Making:
    • Makes thoughtful, well-reasoned decisions; exercises good judgment, resourcefulness and creativity in problem-solving
  • Initiative & Flexibility:
    • Demonstrates initiative, often seeking out additional responsibility; identifies problems and solutions; thrives on new challenges and adjusts to unexpected changes
  • Cooperation & Teamwork:
    • Respectful of colleagues when working with others and makes valuable contributions to help the group achieve its goals
  • Performance category:
  • Knowledge of Position:
    • Possesses required skills, knowledge, and abilities to competently perform the job
    • Training & Development:
    • Continually seeks ways to strengthen performance and regularly monitors new developments in the field of work
  • Performance goals:
    • Set objectives and outline steps to improve in problem areas or further employee development.
    • Overall rating
    • Exceeds expectations
  • Employee consistently performs at a high level that exceeds expectations:
    • Meets expectations
    • Employee satisfies all essential job requirements; may exceed expectations periodically; demonstrates the likelihood of eventually exceeding expectations
    • Needs improvement
  • Employee consistently performs below required standards/expectations for the position; training or other action is necessary to correct performance  
  • Employee is unable or unwilling to perform required duties according to company standards; immediate improvement must be demonstrated
  • Comment on the employee's overall performance.
