For each hazard to which employees in your unit may be exposed (identified in the Work Process Hazard Assessment Form), please list
and describe the job task, the associated hazard (specific chemicals/biological agents/physical hazards), any control measures in
place, and any personal protective equipment used or needed in the table below. Please consult the safety data sheet for products
